nyc adoption agencies

What does it mean when the first name on the birth records is just one sex?
I try to find a job compared with only a date of birth and sex. I was looking through the birth of the late 40's to New York. I have reduced the list of 210 names of an original 300,000. Now I wonder what it means when there is only one kind listed for the name, Smith is, male or female. Does this mean that simply were not named before birth certificate has been issued? Or perhaps died at birth / fetal death? Or should be sent to an adoption agency? Or maybe something else? Any help would be greatly appreciated. Thanks.
Working in a hospital business office, I know there are babies who are rejected and the parents have not decided on the name. As no doubt that was the reason.
Must an adoption be handled through an adoption agency?
